For Interview:

  • Résumé
  • Pre-hire testing is completed on site as part of your interview process (not open access/book)

At Hire:

(be prepared to bring)
  • Professional license and/or certifications
  • Current CPR
  • I-9 self-identification documents
  • Proof of current PPD, chest X-ray or blood TB assay test
  • Proof of level of education, diploma or transcript
  • To protect our patients and other employees, Aveanna performs background checks on all personnel
  • Physical or health statements as required by state regulation
  • Immunization records as required by state regulation
  • Contact information for two references
  • Additional documents based on state regulation (Location staff will clarify with you prior to orientation.)

PDHC Bus Drivers:

  • 7-year motor vehicle record
  • Class D chauffeur driver’s license as required by state regulation

"My son Jeremiah has grown not only physically but emotionally as well with physical therapy.

When Jeremiah was born, we already knew he would have a harder time with simple things such as eating, talking, and walking. Even crawling was going to be a task for him. When we began physical therapy, Jeremiah wasn’t even able to hold his head up on his own at the age of one.

Hope was not lost with this little soldier though. Jeremiah still laughed, smiled and played like any other baby despite his physical impairment. With the help of his physical therapist and his home care nurse, Jeremiah began to crawl and that was one of the happiest days of my life.

Things really went up from there, especially after we moved into our new home. Jeremiah started WALKING! It was a huge shock to me because he just decided to let go, like an angel was guiding him. It was a beautiful sight.

This whole journey with physical therapy has been God sent. I know without his physical therapists, Jeremiah would not be where he is today."

I am thankful for the grace of God in helping me beat a stage 4 terminal cancer diagnosis and remaining cancer free for 15 years now! I am thankful for my family, those living and those that have passed. I am thankful that I can work in the field that I love and help children and their families. I am thankful for my life!!

I am thankful for family. This day, this moment, my brother Tom is dying. I just received word. I'm thankful for prayers, and ask for your prayers for Tom' s health. This day, this moment, my brother Larry is battling cancer. I'm thankful for prayers, and ask you to keep Larry in prayers. I am thankful for all those who love me, me with all my flaws. I am thankful that I am alive!

I'm thankful for my family and my work family. My identical twin sister was diagnosed with breast cancer 5 months ago and has 2 chemo treatments left. She is definitely a strong woman and survivor. I'm thankful to know her tumor is gone and she is winning this fight! I'm blessed to have a job I love and a schedule that let's me have the time I need to take care of my families needs. I am thankful my husband returned home from 30 days of training(military) and I can start to work on the weekends again. I work only when he is home because we have 3 little girls that I stay home with during the week.

I am thankful for my family and friends. I'm thankful to be healthy so that I can help children and their families. I'm very thankful to have been with my current patient for 9 years now.

I’ve worked for this company and I find it to be professional, caring people to its patients and staff. I’m happy to work for them.

I really love being a staff at Aveanna. My scheduller Mr.Mauricio is an awesome guy. This guy is always willing to assist you in your time of need. I also love my Aveanna family who are very welcoming to the staff at their home. This makes the job a lot easier. May God bless Aveanna and everyone who is comitted to make this company to grow.

Today we would like to introduce Home Health Aide Marcia and her Aveanna patient Isamia. Marcia is an incredible caregiver with amazing compassion and care. We asked Marcia to share with us what it’s like to be the caregiver for Isamia.

“It's always a pleasure working with Isamia, or Mimi as we like to call her," Marcia shares. "I love caring for her because I have the knowledge about Mimi’s medical condition that I need to give her the best care.

“During my shifts with Mimi, I encourage her to be independent in her daily activities. I teach her how to wash her hands, dress and undress, feed herself, and say ‘thank you’ when I give her something to eat. She has a good appetite and rarely refuses anything I give her.

“Mimi is a very loving and happy child. She loves when I hug her and is always smiling. I really enjoy caring for Mimi. She has become like family to me.”
